From discharge summary to a nurse in the room
Most nursing starts on the worst possible day — a Friday-evening discharge, a hospital bed to arrange, instructions nobody wrote down clearly. This is the sequence we run.
The discharge is read properly
Your physician reads the summary and turns it into an actual nursing plan — what needs doing, how often, and what to watch for.
The room is made workable
Bed, oxygen or suction arranged if needed. Supplies listed. The family is shown what is where.
Clinical care, and a written log
Wound care, injections, catheter and tube management, vitals. Logged every shift, so nothing depends on somebody remembering.
Reviewed by a physician
The plan is reviewed against progress — shifts reduced as recovery allows, escalated if it does not.
Skilled nursing, and the judgement to escalate
Nursing is not the same as a nursing assistant. Nurses are qualified to perform clinical procedures; assistants help with bathing, feeding and mobility. Many families need both — your Care Manager will tell you honestly which hours need which, rather than selling you the more expensive one.
The fortnight after discharge decides the next six months
It is when wounds get infected, medicines get muddled and readmissions happen. Nursing at home during that window is the least glamorous and most consequential thing we do.
